Interlined 11 line plastic slate consisting of 19 cells per line. Slate fits standard 3x5 index card. 1 side opening to slide the index card in. A hole is provided at one of the corners to attach a stylus toit.

I ordered a device from Amazon that was totally useless. Could not get the bottle in with or without cap. "Fingers" were rock hard to hold the bottle. This is so easy to use.
